Equipment checkout sheet
Make a printable sign-out sheet for tools, laptops and shared equipment: blank rows, or pre-filled with your asset tags and a QR code for each. Choose the columns, print on Letter or A4, and keep a record of who has what.
In the print dialog, set the scale to 100% or Actual size. Choose Save as PDF as the printer to keep a copy.
Getting the most from a checkout sheet
- Keep it where the equipment lives. A clipboard on the tool crib door or the AV cupboard gets filled in; a form in someone’s inbox doesn’t.
- Pre-fill the items when you can. A row per asset, with its tag already printed, shows at a glance what is out and stops two borrowers writing the same drill down differently.
- Ask for a due-back date. It turns “who has the ladder?” into a list of what is overdue.
- Record the condition on return. Damage noticed at the moment it comes back can be followed up while the borrower still remembers what happened.
- File the full sheets. A stack of completed sheets is the only history a paper system has, so keep them together and dated.

At minimum the item, who took it, when, and when it came back. Most teams add the asset tag, a due-back date and a signature at each end, and a condition column if damage is a problem. Choose the columns that match how you actually lend equipment; a sheet with too many boxes stops being filled in.
